Person-Centred Care
Person-centred care gives patients and their families a voice in the design, delivery and evaluation of their care. This approach to care helps improve the person experience, and the quality and safety of care. It also delivers better outcomes and greater value through wiser use of resources.
Patient and family engagement is the cornerstone of the person-centred care approach. People with chronic kidney disease and their families offer unique perspectives on the healthcare system. Their views and experiences with the kidney care system guide and shape our work.
Purpose
Our work will make sure:
- patients and families are empowered to make shared decisions with their healthcare team across the kidney care journey
- patient and family feedback guides regional and provincial quality improvement
- patients and families help plan and evaluate kidney care services
Major Initiatives
- Developed and put in place standardized tools for shared decision-making, encourage self-management and establish goals of care with input from patients and families
- Collect and report on patient experience and outcome measures so we can focus our efforts on improving quality where they are most needed
- Develop formal opportunities for patients and family members to be involved in kidney care system initiatives
Person-Centred Decision-Making
Person-centred decision-making takes place on a continuum. It helps keep treatment decisions in line with a patient’s wishes, values and beliefs. The continuum includes conversations about advance care planning, goals of care and healthcare consent. Having these conversations early and often helps make sure patients and their families are supported appropriately throughout their kidney care journey.
