Indicator | Proportion of patients with end stage renal disease (ESRD) who have received a living or deceased donor transplant in the past 12 months. The treated ESRD population is defined as all patients on chronic dialysis and those who received a kidney transplant. |

Denominator | Patient Cohort: Total of prevalent chronic dialysis patients and patients receiving home dialysis training at the end of the period and patients that received at least 1 kidney transplant in the last 12 months (including pre-emptive transplants) Exclusions: Patients younger than 18 years of old at the time of kidney transplant or at the end of the reporting period for prevalent chronic dialysis patients |
