Kidney Transplant – Indicator Methodology

Topic Description
Indicator Proportion of patients with end stage renal disease (ESRD) who have received a living or deceased donor transplant in the past 12 months. The treated ESRD population is defined as all patients on chronic dialysis and those who received a kidney transplant.

Denominator Patient Cohort:

Total of prevalent chronic dialysis patients and patients receiving home dialysis training at the end of the period and patients that received at least 1 kidney transplant in the last 12 months (including pre-emptive transplants)

Exclusions:

Patients younger than 18 years of old at the time of kidney transplant or at the end of the reporting period for prevalent chronic dialysis patients
Numerator Number of patients that received at least 1 kidney transplant in the last 12 months, including pre-emptive transplants
Data Source(s) Ontario Renal Reporting System (ORRS), Trillium Gift of Life Network (TGLN)
Considerations Kidney transplants are allocated to Regional Renal Programs as follows:
  • Transplants are assigned to the first Regional Renal Program where they were active (based on ORRS) 30 days prior to the kidney transplant and where they stayed for at least 30 days.
  • If the patient was at an Independent Health Facility (IHF) or a Self-Care Facility 30 days prior to the kidney transplant, the patient will be assigned to the Regional Renal Program where the patient was active prior to transferring to the IHF or Self-Care Facility.
  • If the patient first appeared in ORRS within 30 days of the kidney transplant, the patient will not be assigned to an RRP.
