Indicator Proportion of incident chronic dialysis patients with a documented Goals of Care conversation

Directionality Higher is better
Denominator Inclusion Criteria:

Patients newly registered for chronic dialysis or patients transferring into the province for chronic dialysis

Exclusion Criteria:
  • Patients younger than 18 years of old at the time of chronic dialysis registration
  • Patients who recovered kidney function within 30 days of chronic dialysis registration
  • Patients who did not stay in the program for the full observation period (i.e., 90 days from dialysis start) due to death, recovery, transfer out of province, withdrawal, transplant or program transfer greater than 3 days, and did not have a GOC conversation in the observation period.
Numerator Number of incident chronic dialysis patients who had their goals and values incorporated into a documented plan of treatment within 90 days after chronic dialysis registration.
