Advanced Kidney Disease

People with advanced kidney disease are at high risk of progressing towards end-stage kidney disease. Multi-care kidney clinics in renal programs across Ontario provide multidisciplinary care management for these individuals. These clinics aim to slow the progression of end-stage kidney disease and prepare people for renal replacement therapy.

Purpose

Our work helps make sure people with advanced kidney disease will have timely access to multidisciplinary care.

Major Initiative

We define care expectations and accountabilities for the multi-care kidney clinics in a best practices document so they can deliver high-quality and coordinated person-centred care.
