Confirm Diagnosis

Re-measure the person for eGFR and urine ACR 3 months after initial assessment. Select results of both measurements to confirm diagnosis.

The person may have CKD.

Note: At least 2 out of 3 random urine ACRs must be elevated (≥ 3) within 3 months to confirm the presence of CKD.

The person may have CKD and can be followed in primary care.

The person likely does not have CKD.
