Evaluate Risk

Evaluate a person's risk of CKD for the first time. The individual has one or more of these CKD risk factors:

  • Hypertension
  • Diabetes mellitus
  • Cardiovascular disease
  • First degree relative with CKD
  • First Nations, Inuit, or Metis people(s) ≥ 18 years of age
