Integrated Dialysis Care Models

Regional Renal Programs across Ontario are adopting integrated dialysis care models for assisted peritoneal dialysis care.


Assisted peritoneal dialysis care is delivered in patients’ homes and long-term care homes across Ontario. The models aim to improve the patient experience with assisted peritoneal dialysis and provide equitable access to these services.

With these models, the responsibility and funding for peritoneal dialysis assistance rest with the Regional Renal Programs. Having a single point of accountability in each region gives the Regional Renal Programs better oversight of the delivery of kidney care services across all care settings.

We encourage the programs to develop care delivery models that work best for their regions, based on their local circumstances and the characteristics of their patient groups. Customizing the model for each region leads to more seamless, integrated and equitable care for people who choose to dialyze at home.

The integrated dialysis care models are being phased in across the province, with the first ones put in place in early 2018.
